Until 2010, error rate calculations included not only the "improper payments" recovered by CMS or its designees but also recovered funds from a multitude of other sources. Multiple notifications can be made to a practice regarding overpayment refund requests or to produce records (usually a sampling by provider). For very large practices (50 to 100+ providers), the numbers can be enormous.
