I tried repeatedly to re-install the CAS role, re-create the EWS virtual directory and alway came up against the same problem. could not re-create the virtual directory. One thing I didn't try that may be worth a go for some-one in the same position is to completely uninstall IIS and the Windows Process Activation Service and then re-install both. From what I've been reading since re-building my server I believe the issue I faced was an orphaned record in the IIS metadata. Don't know for sure, but may be worth a try.
