I used a rubber eraser to carefully polish each one of the fingers and their connections points on the circuit board. I re-installed the board, and the error is gone. Contributor 30 Answers Re: Fatal Error 121 Hi and welcome to Fixya! "Fatal Error" on your printer display or status monitor simply means a problem has occurred requiring action to clear. Fatal Error explanations are not usually found in the manual. You can check the list of error codes from the link below: http://www.c8600.co.uk/index.asp?printername=C8600&menumode=0&content=errorcodes RECOMMENDED STEPS: First, try turning off your printer then turning it back on. If you're still getting the same error message, make sure the printer is plugged directly into the wall. Often, the issue is power-related. If the problem persists, write down the message including the 3-digit error code number and send an email to "support@okidata.com" or call Oki Data Customer Support (1-800-654-3282) for assis

You will need to either get a technician in to fix it, or get creative like I did and order the part# 43589701 (Gear Coupling A Kit) and then you will have to file out the frame so you can pull the gear assembly out without having to pull the whole printer apart (which is apparently what the techs have to do) Be very careful not to damage any wires while filing the frame hole bigger. Then it is pretty simple to get the gear assembly out and the new one in. This probably cost me like $30 dollars for 2 of these parts and guess what, it broke a second time on me 6 m

Based on the error location I'm going to assume that you have some configuration somewhere (possibly cached) which is declaring an active carrier. share|improve this answer answered Jan 22 '14 at 1:33 benmarks♦ 13k32582 how should I fix this?
