If your passport was issued less than a year ago, you can apply for a new passport with your new name, using form DS-5504. Other Considerations If you find out your birth date is listed incorrectly on your passport, you can get it changed free of charge any time before the passport expires. If your passport was issued more than 90 days prior, you also must submit a new passport photo. United States passport photo requirements state that photos must be 2 inches by 2 inches in size, in full color, and taken against a white or off-white background.
