It looks like they have it set up to use some kind of a default document that then allows them to enter data in the document as a sort of form. When we try to print the document on certain printers, the template part of the page comes out, but the form data does not. So, we

As this is not happening, you finally download the doc and print using evince. Did you try opening the downloaded document in adobe reader and then print it.
