For this I have two tables. mdl_qadt and mdl_qads. I did the teacher part, that is teacher can add question. This is saved to table mdl_qadt. Now student login and select this activity display a form. Here students can post their answer. When posting the answer that give an error: Error writing to database. But the answer and relevant data are inserted into the table mdl_qads. Then why the error shows?? Are you installing Moodle from Softaculous, or doing a manual Moodle installation? Please detail the system that you are using: VPL, Moodle, OS, DB, etc. and any installation detail to keep in mind. You also can set your Moodle in debug mode to get more details of the error. Beware to send the error messages to the log file, not to the HTML page. I installed moodle VPL programming lab plugin and i replaced default server to my server IP.
