This can be caused if there is a fatal error in the script. From PHP Runtime Configuration: Although display_errors may be set at runtime (with ini_set()), it won't have any affect if the script has fatal errors. This is because the desired runtime action does not get executed. Proceed to your domain in Plesk control panel and turn off "Custom Error Documents" under "Physical hosting setup". This will send error message directly to the browser. In my experience, if the event log is empty than the request wasn't correctly routed to the worker process. In one of our recent deployments we saw the app work intermittently with about 50% of the requests randomly failing with the 500 error and nothing in the logs. I suspect something is going wrong with the AppDomain unload that occurs after deployment. Out of curiosity, are you running antivirus in your production environment?
