I see it all the time when setting up or making changes to websites. The good news? It's not your fault. The bad news? There's probably nothing that you can do. Undo can hekp the user back out of a fix they don't want. Offer to fix the problem, with the most commonly accepted fix as the default choice. Inform the user of the problem and advise the user what steps to take next. Inform the user of what happened. Do nothing because the failure is catastrophic. In the case of a server error, you can't necessarily do much more than option 4, above.
