The other Answerer made a silly mistake about percentages. It can exceed 100% if one measures badly. The measurer might have measured it as 5 or 50 also. Usually it's more a matter of using inappropriate tools. Like measuring a tablespoon with a gallon jug. The accuracy of the gallon jug is +/- 1/2 gallon. That vastly exceeds the tablespoon of whatever that you are measuring. Huge error, even without a foolish reading like the measurer for the problem recorded. (Actually, the problem could have happened just like what I described with the jug, but I wanted to emphasize the role of operator error in things, not just inappropriate tools.)
