One to a few times during the work day, seemingly at random, I am getting a log entry of "Login failed for user 'OurDomain\mfreeman'. [CLIENT: 192.168.0.48]". This is paired with a log entry of "Error: 18456, Severity: 14, State: 16." at the same time. This is my own login (which has the sysadmin role in the instance), and it is coming from my workstation. I captured an example of this with sql server error 18456 state 38 Profiler and found that it is for the master database (which is online and not having any troubles -- I can run queries against it in SSMS just fine) and is coming from an ApplicationName of ".Net SqlClient" (which could be anything). I am not getting any visible errors in any applcations (such as VS2012 with SSDT and Report Builder) or SSMS add-ins that I am using (ApexSQL Complete, SSMS Tools, Dell Spotlight Essentials). This could be because the database has been removed, renamed, or is offline (it may be set to AutoClose). This state does not indicate a reason in the error log. Resolve by fixing the missing database, or changing the login's default database using ALTER LOGIN, e.g. ALTER LOGIN [your_login] WITH DEFAULT_DATABASE = [valid_database]; This state may also be reported if the user's default database is online, but the database they explicitly requested in the connection string is not available for the reasons stated above. SELECT name FROM master..sysdatabases EXEC sp_databases obviously some thing is wrong some where? so how do I resolve this login error? even if I login correctly, how come I cannot view the master database? what must I do to be able to view the master database? I tried re-deploying it but got a message stating that Error: 18456, Severity: 14, State: 16 an internal error occurred on the report server If checked to see if the default database has been renamed, removed or if it's offline and in our case neither is applicable as the default DB for the user used is master. Being SQL Server 2005 I also checked to see if the database being called has been renamed, removed or offline and neither holds true here either. Also, I verified that Database is not set to Auto-Close. Also SSL is not disabled on the server. Any other suggestions?
