You may have tried to recreate the user profile or the user's Outlook profile and thought that it worked…but it didn't because it only ran on the first try and crashed every time after that. It is hinted at with this Microsoft KB article that you might have a mismatched olmapi32.dll file. This may be because you had a previous version of Outlook installed on the system. But as you can see from the event log entry, the version numbers of outlook.exe and olmapi32.dll seem to match. I haven't actually tried repairing the Office installation but I do have another solution…install a Microsoft update that updates olmapi32.dll.
