When you install Adobe Flash and allow it to auto update, it creates a scheduled task in the Windows Task Scheduler (XP - Control Panel double-click the Scheduled Tasks folder). This could have been one place you could check for any information regarding the failed attempt. Also event viewer may provided some details but usually you see what is reported in the error which appears. After removing and re-installing flash, the scheduled task should no longer exit.
