BEND, Ore. - A settlement has been reached between the family of Loretta Macpherson, 65, http://www.ktvz.com/news/bend/settlement-reached-in-medication-error-death-at-st-charles-/69122627 and St. Charles in Bend after a medication error at the hospital led to the death of the Sisters resident in late 2014, an attorney and hospital officials confirmed Tuesday.The family's attorney, Jennifer Coughlin said Tuesday she cannot disclose the amount of the monetary settlement, which was reached last Thursday.Macpherson died at the hospital in December 2014 after a St. Charles employee put the wrong medication into her IV bag.Instead of an anti-seizure medication, Macpherson the Sisters resident was given a paralyzing agent, causing irreversible brain damage and cardiac arrest. Two days later she was taken off life support.Coughlin said Macpherson's two sons, 28-year-old Mark Macpherson and 33-year-old Pete Macpherson, didn't want to prolong the process any further and opted for a settlement.The lawyer added that the settlement brings closure for her sons, but won't undo the loss they suffered.Deschutes County District Attorney John Hummel told NewsChannel 21 on Tuesday the criminal investigation of the incident is still ongoing.St. Charles Health System issued the following statement:"St. Charles Health System and the family of Loretta Macpherson have reached a settlement agreement in recognition of the loss of their mother due to a medication error that took place at St. Charles Bend in December 2014."The terms of the agreement are confidential, but it will result in a financia

Medication mishaps can occur anywhere in the distribution system: prescribing, repackaging, dispensing, medication error administering, or monitoring. Common causes of such errors include: poor communication, ambiguities in product names, directions for use, medical abbreviations or writing, poor procedures or techniques, or patient misuse because of poor understanding of the directions for use of the product. In addition, job stress, lack of product knowledge or training, or similar labeling or packaging of death medication error a product may be the cause of, or contribute to, an actual or potential error. CDER began receiving reports of medication errors in January 1992, when the U.S. Pharmacopeia began forwarding reports to the FDA. To evaluate and recommend appropriate action on these reports, the Medication Errors Subcommittee was formed in June 1992. In November 1993, the Agency began evaluating and coding MedWatch reports for medication errors and publicly stated that physicians and other health care professionals could report medication errors directly to the FDA through the MedWatch program. CDER responsibilities are not completed when the safety and effectiveness of a drug product are determined. The Center also has the responsibility for helping to ensure the safe use of the drugs it approves by identifying and avoiding proprietary names that contribute to problems in the prescribing, dispensing, or administration of the product. Because early identification of a potential confusing proprietary name is crucial, CDER reviews these proposed names, prior to approval of a new drug application, by
