On your motor there is a wheel with holes in it. On top of that is the optical sensor. The sensor has to line up perfectly with the holes for the sensor to read correctly. The sensor counts the holes to know how fast the treadmill is going. Check to make sure the sensor is plugged in to the board and in itself. If the sensor is properly aligned it should work perfectly. Make sure the bracket for the sensor is not bent.
