This chapter summarizes the evidence base on rates of medication errors; preventable adverse drug events; and failure to prescribe medications for which the evidence supports the ability to reduce morbidity and mortality in hospital, nursing home, and ambulatory settings. to reduce the risk of medication errors. This chapter also summarizes the evidence base on these costs. As noted in Chapter 1, the committee’s charge encompassed developing estimates of the incidence, severity, and costs of medication errors and evaluating alternative approaches to reducing such errors in different settings.
