prevent their recurrence. One way to learn from errors is to establish a reporting system. Reporting systems have the potential to serve two important functions. They can hold providers accountable for performance or, alternatively, they can provide information that leads to improved safety. Conceptually, these purposes are not incompatible, but in reality, they can prove difficult to satisfy simultaneously.Reporting systems whose primary purpose is to hold providers accountable

are "mandatory reporting systems." Reporting focuses on errors associated with serious injuries or death. Most mandatory reporting systems are operated by state regulatory programs that have the authority to investigate specific cases and issue penalties or fines for wrong-doing. These systems serve three purposes. First, they provide the public with a minimum level of protection by assuring that the most serious errors are reported and investigated and appropriate follow-up action is taken. Second, they provide an incentive to health care organizations to improve patient safety in order to avoid the potential penalties and public exposure. Third, they require all health care organizations to make some level of investment in patient safety, thus creating a more level playing field. While safety experts recognize that Page 87 Share Cite Suggested Citation: "5 Error Reporting Systems." Institute of Medicine. To Err Is Human: Building a Safer Health System. This section explains how to create a cmdlet that reports nonterminating errors from its input processing methods. For nonterminating errors (as well as terminating errors), the cmdlet must pass an ErrorRecord object identifying the error. Each error record is identified by a unique string called the "error identifier." In addition to the identifier, the category of each error is specified by constants defined by a ErrorCategory enumeration. The user can view errors based on their category by setting the $ErrorView variable to "CategoryView".For more information about error records, see Windows PowerShell Error Records.Topics in this section include the following:Defining the CmdletDefining ParametersOverriding Input Processing MethodsReporting Nonterminating ErrorsCode SampleDefining Object Types and FormattingBuilding the CmdletTesting the CmdletDefining the CmdletThe first step in cmdlet creation is always naming the cmdlet and declaring the .NET class that implements the cmdlet. error or hazard to ISMP. If you are a CONSUMER, please click on the orange button https://www.ismp.org/orderforms/reporterrortoismp.asp below if you are ready to report an error or hazard. If you are a HEALTHCARE PRACTITIONER, you can report the error or hazard to ISMP using one of two secure methods: 1) Report to the ISMP National Medication Errors Reporting Program (MERP) or the ISMP National Vaccine Errors Reporting Program (VERP) These are confidential, voluntary reporting programs error reporting operated by ISMP to learn about the causes of medication and vaccine errors. After you submit a report, ISMP staff will follow up with you to ask additional questions to clarify what went wrong and to identify the causes and factors that contributed to the reported event. The report will also be forwarded in confidence to the error reporting system US Food and Drug Administration (FDA) and, when applicable, to product vendors to inform them about pharmaceutical labeling, packaging, and nomenclature issues that may cause errors by their design. Your name, contact information, and location will NOT be submitted to FDA or product vendors without your permission, and identifiable information will NOT be disclosed outside of ISMP. Click on the appropriate button below if you are ready to report an error or hazard to the ISMP MERP or ISMP VERP. Click here to learn more about ISMP reporting programs. 2) Report an adverse event to ISMP as a Patient Safety Organization (PSO) You may prefer to report an adverse medication or vaccine event to ISMP as a federally certified Patient Safety Organization (PSO). As a PSO, federal legal protection can be provided for certain patient safety information prepared within your patient safety evaluation system and submitted to ISMP. The protected information, called patient safety work product, includes information you provide about why the error happened or why the ha
