Indiana hospital errors in 2014 were the highest in nine years of reporting.(Photo: Lukasz Panek / Getty Images / iStockphoto)Indiana once again has set a record for the number of medical errors that occurred in the past year.In 2014, hospitals and other health care facilities reported a total of 114 preventable adverse medical events, three more than in any of the other eight years since the Indiana State Department of Health started gathering the data. In 2013, the total was 111, surpassing the previous high of 107 in 2010. When the state first announced plans to start collecting data on 28 preventable things that can go wrong in hospitals and other health care facilities, such as fatal falls or leaving something inside a patient after surgery, experts expressed caution with comparisons.
